Abstract: : This study aims to determine the profile of the quantity of antibiotic use based on the DDD / 100 patient-days method and the quality of antibiotic use based on the percentage of suitability (indication, time of administration, duration, dose, and route of administration) of ASHP Therapeutic Guidelines, Antibiotic Use Guideline (AUG) Hospital, and Hospital Formularium, as well as profiles Surgical Site Infection (SSI) in surgical patients in the West Nusa Tenggara Provincial Hospital (WNT PH) in the January-June 2019 period. This research is an observational study analyzed descriptively with the direction of retrospective data collection using medical record data. The research sample was patients who underwent surgery and met the inclusion criteria, obtained as many as 323 people. The results obtained from this study were the quantity of antibiotic use in the January-June 2019 period dominated by the antibiotic ceftriaxone (J01DD04) with a total DDD / 100 patient-days value in pre-surgery that was 77,655, on surgery 87,31, and post surgery 93,65 . The suitability of prophylactic antibiotic selection based on ASHP guidelines of 1.9%, AUG 15.5%, and Formulary of NTB Provincial Hospital 100%, while the suitability of duration, time of administration, dosage, and route of administration based on ASHP were 19.2%, respectively, 42.7%, 1.5%, and 100%. The research samples that experienced SSI were 2 of 323 samples (0.62%) with the results of bacterial growth namely Proteus sp., Staphylococcus aureus, S. epidermidis, and E.coli. The overall sensitivity test of the bacteria that causes SSI was found to be still sensitive to antibiotics such as chloramphenicol, amoxicillin-clavulanic acid, vancomycin, cefoxitin, and oxacillin. It can be concluded that the use of antibiotics in surgical patients at RSP NTB is entirely in accordance with the applicable formulary, but an update of the existing formulary is needed to follow the development of bacterial sensitivity
