Abstract: Abstract Background: Hypertension is a cardiovascular disease that can attack anyone and cause death. The increase in hypertension is still largely due to lack of knowledge. Knowledge about hypertension is one of the factors for someone to adhere to a hypertension diet. Someone who has a high level of knowledge will have compliant behavior in implementing a hypertension diet. Purpose: To determine the relationship between level of knowledge and adherence to a hypertension diet in the elderly. Method: Quantitative with correlative descriptive using a cross sectional approach. This research was conducted at the Pajang Community Health Center in December 2023. The data collection technique used a questionnaire which was distributed directly to respondents. The research sample consisted of 33 respondents using total sampling. Data analysis used univariate and bivariate with the Spearman technique. Results: Based on research that has been conducted, it was found that 24 (72.7%) respondents were in the category of lacking knowledge and in the category of non-compliance with the hypertension diet were 27 (81.8%). Spearmen correlation test results p-value 0.016 < p 0.05. Conclusion: There is a relationship between the level of knowledge and compliance with a hypertension diet in the elderly. Suggestion: It is hoped that the elderly can increase their knowledge about hypertension. If knowledge about hypertension increases, compliance with the hypertension diet will increase. For health workers on duty, they should provide health education to the elderly more regularly and can also provide motivation for the elderly to live a healthy life by maintaining a healthy diet and regular physical activity.
